MEMBERSHIP BENEFITS

Members receive monthly access to premium wellness services, including: IV Rejuvenation Therapy, IV Enhancements, and Purified Oxygen Therapy. Services are offered at a discounted rate based on the selected membership tier.

payment terms

membership is billed monthly to the credit card entered at the bottom of this agreement.  members are responsible for updating payment information to avoid service interruption.  declined charges not resolved within 5 days may incur a $35 non-sufficient funds (nsf) fee. 

membership termination and changes

memberships can be cancelled at anytime, but monthly fees are non-refundable and will not be prorated.  provider reserves the right to modify membership offerings, pricing, or terminate membership at anytime.

maintain accurate billing information.  adhere to appointment policies.  respect the terms outlined in this agreement.
